The Order Form and the current price lists (both In-Print and Out-of-Print) are available as PDF files. To view and print these files, you will need Adobe Acrobat Reader. If you do not have this program, you can download a free copy by clicking on the “Get Acrobat” icon at the bottom of this page.

ORDERING INSTRUCTIONS:
Print the Order Form. Indicate the quantity of publications desired and the total cost of the volume(s) ordered next to the volume number. Enter the order subtotal in the space provided at the bottom of the form. Add the appropriate postage and handling charge. Orders within Illinois must also add 6.25% sales tax in addition to the postage/handling fee. For fast delivery and foreign shipping rates, contact Kathy Lundeen at tktdc@siu.edu. Clearly print your name and the address where the order should be mailed. Return the order form to the address below with payment for the total amount due. Payment is due in U.S. currency, U.S. bank draft, or international money order.
