Important Notice for students who have applied for OPT
This message is for all international students from SIUC that applied
for OPT for this year.If you have already sent the information that we need
you can disregard this message until you change your address or place of
employment.
The United State Citizenship and Immigration Services (USCIS) requires
that the Office of International Programs and Services/ International Students
and Scholars provide them with the following information for all students that
have applied for and received Optional Practical Training (OPT)
1. Current mailing address
2. A photocopy of the front and back of your Employment Authorization
Document (EAD card)
3. The name of your employer
4. The location of your employer
If any or all of these items are missing in our records you will be
considered to be Out of Status with USCIS as an international student
and ineligible for employment.You may also be subject for arrest
and deportation.
We would also like to have a copy of the I-797 Notice of
Action - receipt – that you received before your EAD arrived.
You can send these items in the mail, you can fax them to

information so that you maintain your status with USCIS
