Immunization Requirement


To ensure continued eligibility to attend SIUC, students must comply with the Illinois Mandatory Immunization Law. The law requires that students provide documentation with a physician or nurse signature that they have received the following immunizations:

  • One dose of Diphtheria and Tetanus (DT) with the most recent dose having been received within 10 years.
  • Two doses of Measles (Rubeola) administered with the live virus vaccine on or after first birthday.
  • One dose of Rubella vaccine received on or after first birthday.
  • One dose of live Mumps vaccine received on or after first birthday.
  • TB (Tuberculosis) screening conducted at SIUC Health Center is required for international students.

A copy of an existing health record from your high school, family physician, local health department or military records is acceptable. Send documentation directly to:

Or you may have your health care provider complete the required Immunization form at: http://www.siu.edu/~shp/downloads/Forms/ImmunComp.pdf

If you cannot provide proof of immunization, you must arrange to have the immunizations done here at SIUC. Please contact our Student Health Center upon your arrival at SIUC. For appointments and information about immunizations, please call (618)453-4454.

Please Note: A $25.00 late compliance fee will be assessed to students who fail to provide proof of immunity or begin to receive the necessary series of immunizations by the end of the seventh week of the semester. This is NOT refundable.
