COMMITTEE APPROVAL FORM
This form must first be discussed with the Chair of your thesis committee; it must be signed by your chair and then submitted to the Graduate Director.
NOTE: THIS FORM MUST BE TYPED
THESIS/DOCUMENT APPROVAL FORM
This form must contain a signed statement by your committee chair before it is submitted to the Graduate Director WITH FOUR COPIES.
Your Thesis or Document topic must be apporved by the graduate committee at least one semester before it is submitted to the Graduate School.
APPLICATION FOR COMPREHENSIVE EXAM FORM
A copy of this completed form must be delivered to each member of your thesis committee AND the Graduate Director at least three weeks prior to your comprehensive exam.
You are responsible for scheduling your exams. The exact times for your exams will should be discussed and planned with your Chair.
DOCUMENT APPROVAL FORM
After your document is approved by your committee, this signed form will serve as the title page for your document.
