Insurance Benefits FAQ

 

1. What information is needed to submit a claim?
Claim Form,Primary Insurance Information Form,Insurance acceptable bill, and if the student has primary insurance, an Explanation of Benefits MUST be submitted with the claim.

2. How long does it take to process a claim?
Approximately 4 weeks from the date all necessary Information is received.

3. Is the student medical insurance coverage primary or secondary?
Secondary, the Student Insurance Plan is always secondary to other medical insurance policies. Exceptions apply to Champus and Dept. of Human Services.

4. How does a student apply for a refund of the Student Medical Benefit Extended Care Fee?
Application for a refund must be made by the Friday of the second week of Fall & Spring Semesters and by the Friday of the first week of the Summer Session (Only if summer is the first session or only session) whether or not all tuition and fees are paid. Application must be made each semester that a refund is desired. To obtain a refund, please bring a copy of a wallet ID card and the Schedule of Benefits from your current insurance policy. This must also include information regarding the major medical deductible of that policy which must be $1,000 or less to Qualify for the refund. Application can be made at the Student Health Center Student Medical Insurance Office.
